Terms of Service

These Terms of Service ("Terms") govern your access to and use of the EOB Decoder website and the EOB Decoder early-access product (together, the "Service"), provided by EOB Decoder ("EOB Decoder," "we," "us"). By using the Service, you agree to these Terms.

1. Acceptance of these terms

By joining the waitlist or otherwise using the Service, you confirm that you have read and agree to these Terms. If you do not agree, do not use the Service.

2. Eligibility

The Service is intended for United States medical practices and their authorized staff. By using it, you represent that you are at least 18 years old and are authorized to act on behalf of your practice.

3. What the Service is

EOB Decoder is a drafting-assistance tool that helps practices turn claim denials into appeal letters that quote relevant coverage policy. The Service is currently in early access (beta), is provided "as is" and "as available," and may change, be interrupted, or be discontinued at any time.

EOB Decoder assists with drafting appeals. It is not legal, medical, or billing advice, and it is not affiliated with CMS or any insurer. You are always the sender: you review, edit, and send every appeal yourself, and you are responsible for the accuracy of each letter and for what you submit to payers.

4. No guarantee of outcomes

Appeal results depend on the facts of each claim, the payer, and applicable policy. We do not guarantee that any appeal will be accepted, that any claim will be reimbursed, or that the Service will produce any particular result.

5. Early access — synthetic claims only

During early access, you agree to use the Service with synthetic (test) claims only. Do not submit real patient data or protected health information (PHI) to the Service during early access. When the product later supports PHI, it will do so under a signed Business Associate Agreement (BAA) and appropriate safeguards, and additional terms may apply.

6. Acceptable use

You agree not to:

  • Submit real PHI during early access.
  • Use the Service for any unlawful, misleading, or fraudulent purpose.
  • Attempt to disrupt, reverse engineer, or gain unauthorized access to the Service.
  • Misrepresent a drafted letter as independent legal or medical advice.

7. Your responsibilities

You are responsible for reviewing every draft for accuracy, for keeping your account credentials secure, for complying with your payers' requirements and applicable law, and for the content of any appeal you choose to send.

8. Intellectual property and your content

EOB Decoder owns the Service, including its software, design, and content, excluding the materials you provide. You retain ownership of the inputs you provide and the appeal letters you generate and send ("Your Content"). You grant us a limited license to process Your Content solely to provide and improve the Service.

9. Fees

The website references per-appeal pricing for the product. Early-access terms and pricing may differ from general-availability pricing and may change. Any applicable fees will be presented to you before you incur them.

10. Disclaimer of warranties

To the fullest extent permitted by law, the Service is provided "as is" and "as available," without warranties of any kind, whether express or implied, including warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement.

11. Limitation of liability

To the fullest extent permitted by law, EOB Decoder will not be liable for any indirect, incidental, special, consequential, or punitive damages, or for any lost revenue, denied or unreimbursed claims, or lost data, arising out of or relating to your use of the Service.

12. Indemnification

You agree to indemnify and hold harmless EOB Decoder from claims and costs arising out of your use of the Service, the appeals you submit, or your breach of these Terms.

13. Governing law

These Terms are governed by the laws of the United States and the applicable state in which EOB Decoder operates, without regard to conflict-of-laws rules.

14. Changes to these terms

We may update these Terms as the Service evolves. When we do, we will revise the effective date above. Continued use of the Service after an update means you accept the revised Terms.
